Another would definitely be the Thetis, which sank during launch, was later salvaged and renamed HMS Thunderer, and finally was sunk by an Italian corvette. That's all I have off the top of my head. K-129 (I wrote a whole article on that affair for the SubCommittee Report), the early Russian November class (K-3, IRCC) reactor accident, the first Alfa (reactor coolant freeze up) are all possibilities. There was also a Russian in port submarine reactor explosion which made quite a mess. The British submarines that ran on hydrogen peroxide had their share of woes. Squalus, Thresher, K-19, and Kursk are all well covered in previous documentaries. It covers 1774 to 1986 and ISBN is 0-436-18740-X. It was a good read and I was particularly impressed by the tale of the Japanese captain of Submarine No.6 in 1910 who sat down in the knowledge that he would never be rescued to write an account to help with the diagnosis of what led to the disaster. He survived 16 hours but the boat was only raised the following day, which was too late for him and his crew.
